CT Wound Physicians Group, PLLC provides structured, protocol-driven wound management services within skilled nursing and rehabilitation facilities throughout Connecticut.

The practice operates on a systems-based clinical framework emphasizing organization, consistency, and measurable outcomes. Weekly physician-led rounds are conducted in collaboration with facility nursing leadership to ensure accurate staging, appropriate debridement, evidence-informed local wound management, compression and offloading strategies when indicated, and timely escalation for vascular, infectious, or surgical concerns.

All documentation is completed within the facility’s electronic medical record using a standardized format that supports regulatory readiness, continuity of care, and defensible clinical decision-making.

The care model is designed to integrate seamlessly into existing facility workflows while utilizing the facility’s formulary whenever appropriate. Services are provided at no cost to the facility, with reimbursement occurring directly through Medicare Part B and participating payers.

Dr. Krystl George-Ortiz is a Connecticut-licensed physician and the Founder and Medical Director of CT Wound Physicians Group, PLLC. Her clinical focus is dedicated to post-acute wound management within skilled nursing and rehabilitation environments.

Her methodology centers on structured assessment protocols, disciplined weekly follow-up, early identification of deterioration, and clear escalation pathways to support limb preservation and efficient healing trajectories.

As Medical Director, Dr. George-Ortiz oversees clinical standards, documentation structure, and collaborative integration with facility leadership to ensure consistent, accountable wound management delivery.
